Applications and Forms
- Release of Information (English)(Spanish)
- An applicant must specify an agency or organization on the Release of Information form and provide a signature. The agency or organization can contact the county on the applicant’s behalf.
- This is the Calfresh Application form that all applicants can print and send in via mail.
- This form must be submitted (typically) 6 months after initial application in order to renew and continue receiving CalFresh benefits.
- This form must be submitted (typically) 1 year after initial application in order to renew and continue receiving CalFresh benefits.
- If an applicant would prefer to receive communication from the county in a language other than English, they may specify their preferred language by filling out and submit this form.
Verification Forms
- Employment Verification CF 0721 (English)(Spanish)
- If pay stubs or other forms of employment verification are unavailable, an applicant may fill out this form as verification of income from an employer.
- This form must be filled out by the applicant’s sponsor. It must be uploaded during initial application, renewals, and recertifications.
- This form may be filled out as verification of self-employment income and expenses if other documentation is not available.
- Sworn statements may be submitted as verification for an applicant when other documentation is unavailable.
